Most health care providers will file claims for you. However, if you need to submit a claim, please write your WPS Member Number (as found on your ID card) on each page of the claim and mail it to "WPS Claims" at our address.
Note: do not send any confidential information to the below fax address. If you have a question regarding your private health information, please use the "Send a Privacy-Protected Message" method instead (see below).
