Create Your Own Provider Directory
Don't forget to verify your provider is in network!
To verify a particular provider participates in the network when you receive services:
- Call the provider directly or,
- Check the online directory for the day you receive services or,

Call the Members Service Number shown on your health plan ID card.
Step 1
Enter your search criteria in Find a Doctor, and press "Continue.
Step 2
When you see the list of providers on the results page, look to the top
right of the screen and click on the link titled “Print PDF Directory”.
Step 3
The next screen is an optional cover page. Enter your cover page information, then press "Continue".
Find a Doctor will generate a provider directory reflecting your search results which you
can then download or print.
