Request Letters
What do the requests for medical records from the CERT Documentation Contractor look like?
The CDC will request records by sending an initial request and three subsequent reminders. Please access the following links to review examples of these letters.

If the records are not received within the allotted timeframe, the claims will be denied and payment recouped for all services billed on the claim. Additionally, failure to submit the records may also result in referral to the Office of Inspector General.
