Submitting Medical Records When Requested

When medical records have been requested, the following information is required to be attached to the top of the medical records:

• Additional Development Request Letter
• Completed Remote coversheet
• Letter requesting medical records

Without the cover letter, we cannot indicate the reason for the medical records. These records will be returned to you.
