NPI Information
As of June 5, 2006, the National Provider Identifier (NPI) is required on all Centers for Medicare & Medicaid Services (CMS) 04/06 855 Provider Enrollment Forms. Wisconsin Physicians Service (WPS) Medicare and other carriers will require one of three forms of documentation to validate the NPI.
1) A copy of the NPI notification letter. The letter is sent by Fox Systems, INC., the NPI Enumerator, to providers who apply for a NPI using a paper application. The letter contains the logo of Fox Systems, Inc. as well as the CMS logo.
*Note: NPI notification letters are not sent on CMS letterhead.
2) A copy of the NPI e-mail notification. The e-mail is sent by the NPI Enumerator to providers who enroll via the Website. The e-mail does not contain specific logos.
3) A copy of the NPI notification from an Electronic File Interchange (EFI) organization. This notification may be a letter or e-mail format, either of which will be accepted as documentation of the provider's NPI.
Providers who do not have the original notification must contact the enumerator, Fox Systems, to receive a second notification. A screen print from the secured enumerator system is not acceptable proof of NPI for the Medicare enrollment process.
