Slide 94 discusses discharge management services. When researching questions from physician offices, we discovered the need to clarify the information we provided on discharge management services. We stated in this presentation that only the admitting physician could perform the discharge management services. The Centers for Medicare & Medicaid Services (CMS) Internet Only Manual (IOM) Publication 100-04, Chapter 12, Section 30.6.9.2, indicates the attending physician of record or a physician acting on behalf of the attending physician may bill the discharge management codes. Anyone else providing services would submit a subsequent hospital care code.

We also stated a discharge visit could not be shared/split between the physician and the non-physician practitioner. The CMS IOM Publication 100-04, Chapter 12, Section 30.6.13.H, indicates a physician and non-physician practitioner can do a discharge management service as a shared/split visit.
