What do I need to provide when requesting a therapy cap exception?

In order to request a therapy cap exception, the justification for continued PT/OT/SPL treatment must include objective evidence or a clinically supportable statement(s) that includes:
- The patient's condition has the potential to improve or is improving in response to therapy
- Maximum improvement is yet to be attained
- There is an expectation that the anticipated movement is attainable in a reasonable and generally predictable period of time
