2008 CMS-855 Medicare Enrollment Applications - Revised Deadline
The Centers for Medicare & Medicaid Services (CMS) revised the CMS-855 Medicare enrollment applications in February 2008. The current versions of the applications are available on the CMS Provider Enrollment Website at http://www.cms.hhs.gov/CMSForms/CMSForms/list.asp

Applications received in our office on and after December 31, 2008, must be submitted on the 2008 versions of the forms. The 2006 versions of the forms will no longer be accepted and will be returned to the applicant effective January 1, 2009. Although the 2006 versions of the applications will continue to be accepted on an interim basis, providers are encouraged to discontinue their use as soon as possible and to submit the 2008 applications that are on the CMS Website.
