What are examples of statements to use on an ABN?

Medicare has stated that providers cannot use a general statement on the Advanced Beneficiary Notice (ABN.) The general statement does not provide enough information to allow a beneficiary to make an informed decision.
The statement "Medicare may not pay" is deemed too general. Please do not use this statement.
Here are examples of statements that can be used:
- Medicare Part B usually does not pay for this many visits or treatments.
- Medicare Part B usually does not pay for this service.
- Medicare Part B usually does not pay for such an extensive procedure.
- Medicare Part B usually does not pay for this equipment.
- Medicare Part B usually does not pay for this lab test.
Be sure the ABN is specific to the date of service and procedure being billed along with the proper statement. There is no need to provide individual ABNs for a series of services; however, dates of service must be listed individually.
