Health Management supports the doctor-patient relationship and plan of care, emphasizes control of the disease and complications, and uses cost-effective, recommended practice guidelines and patient self-management.
Shared decision making is a joint process that allows patients and their doctors to make health care decisions together. WPS Health Insurance has decision tools available for you throughout our website to use when making decisions about your health and care. You and your doctor can review these tools together to get an accurate picture of the risks versus benefits of your treatment options.
Asthma is a chronic condition in which your airways narrow and swell and produce extra mucus. People with asthma have highly sensitive airways that become inflamed and narrow easily, leading to coughing, chest tightness, and difficulty breathing. Hypertension (high blood pressure)
People with high blood pressure are at risk for serious health problems such as stroke, heart attack, heart failure, kidney failure, and sexual dysfunction. Lowering blood pressure can help reduce these risks, extend life expectancy, and improve quality of life. The Hypertension Health Management Program offers members information, coaching, and resources to help improve high blood pressure and enjoy better health.
