The 2008 Milwaukee County Preferred Provider (PPO) Plan
This plan features comprehensive benefits with deductibles and coinsurance at both in- and out-of-network providers.
Please note that this plan does not cover services for Behavioral Health. Behavioral Health providers are listed in the Statewide or National Networks, but disregard these listings. You'll need to obtain providers for Behavioral Health services through MHN/HMC at 800-472-4992.
